The Medical Conditions Checklist (MCC) helps us understand your family’s openness in terms of a child’s age, gender, and medical condition(s), and will let us know what countries may be a good fit for your adoption journey.

Your MCC is stored electronically and you may come back to revise it anytime you wish! Just click the same button above to log in again with your password to view, print, or make changes to your MCC.

Make sure to use THIS link to log in to your MCC and not your general CCAI log-in information from our home page, as the MCC system is separate from that of your general CCAI adoption account.
Yes! Click the “Create/Update Medical Conditions Checklist” button, or click here and sign in with the email address and password you used to originally create your MCC. You can make any changes you like, as frequently as you wish, and it will NOT change the date of your original MCC submission. Please note – if you wish to update a current MCC on file, make sure to use the SAME email address and password as you used to create it (just call us if you need help!) and select the option to “Update your Medical Conditions Checklist”. Please do NOT select “Fill out a NEW Medical Conditions Checklist” if you already have an MCC on file.

CCAI asks that your family begin your adoption process (with a formal application to CCAI) within one year of submitting your MCC and current, approved home study. If you do not choose to start the adoption process at that time, we’ll remove your MCC from our files and will welcome you to submit a new one when you are ready to begin the adoption process and/or submit a new approved home study to CCAI.
Once you are matched and complete an adoption, the MCC associated with that adoption is no longer active. To adopt again, you’ll need to submit a new MCC, to be filed as of the new submission date.
You can still log in with your email address and password you used before, then just click the link to create a NEW MCC.
